Classifications of Residency: 

 

 

 

For residents that have recently completed the purchase of an existing home in Sunnyvale ISD boundaries

Please navigate to your child's campus homepage and complete all registration paperwork and follow the guidelines below for providing proof of residency:

  1. A copy of the Final HUD Settlement Statement received from your Title Company, including signature pages (must be signed by both buyer and seller)
  2. A copy of any utility deposit receipt; or a copy of your most current water, gas or electric bill

 

For new residents with a signed/dated purchase contract to build a new home in Sunnyvale ISD boundaries OR an existing home in Sunnyvale and have not yet moved into the home

You must return the requested documents below to Sunnyvale ISD Student Services Department, 417 E. Tripp Road, Sunnyvale, TX 75182, before being able to enroll your child at the campus:

  1. Completed Attendance Request out of District Form (2017-2018) Form A
  2. A copy of a valid, fully executed, real estate earnest money contract to purchase with closing date or contract to build with date on which construction began (when slab has been poured) and closing date. If contract does not include date when construction began (slab/foundation poured) please submit a letter from the builder with the anticipated date of the pour.  You will also be required to submit a letter from the builder confirming the date of the slab/foundation pour after it has occurred.
  3. A copy of enrolling parent/guardian's driver's license

Once you have closed on your home, please submit the following as soon as possible as the required final proof of residency:

  1. A copy of the Final HUD Settlement Statement received from your Title Company, including signature pages
  2. A copy of any utility deposit receipt; or a copy of your most current water, gas or electric bill

 

NOTE: The following classifications are valid for one school year only and must be renewed annually during the summer prior to the first day of school to ensure continuous, uninterrupted enrollment of student.

 

For new residents renting in Sunnyvale ISD boundaries

You must return the requested documents below to Sunnyvale ISD Student Services Department, 417 E. Tripp Road, Sunnyvale, TX 75182, before being able to enroll your child at the campus:

  1. Completed Attendance Request Lease Agreement Form (2017-2018) Form B
  2. A copy of a valid, fully executed lease agreement (must be complete agreement with signatures from both tenant and landlord)
  3. A copy of the current water, gas or electric bill or any utility deposit receipt
  4. A copy of enrolling parent/guardian's driver's license

 

For parent and student residing with another family in Sunnyvale ISD boundaries (Family in Residence)

You must return the requested documents below to Sunnyvale ISD Student Services Department, 417 E. Tripp Road, Sunnyvale, TX 75182, before being able to enroll your child at the campus:

 
From Parent:

  1. Completed Attendance Request -Family or Student in Residence Form C (2017-2018)
  2. Copy of enrolling parent/guardian's driver's license

From Sunnyvale ISD Resident:

  1. Completed Completed (2017-2018) Affidavit of Student Admission Form -Residing with parent (one per student-needs notarization) Form D
  2. Copy of most recent property tax statement and a copy of current water, gas or electric bill
  3. Copy of Sunnyvale ISD resident driver's license 

 

For Student ONLY residing with another family in Sunnyvale ISD Boundaries (Family in Residence)

You must return the requested documents below to Sunnyvale ISD Student Services Department, 417 E. Tripp Road, Sunnyvale, TX 75182, before being able to enroll your child at the campus:

 

From Parent:

  1. Completed Attendance Request-Student in Residence (2017-2018) Form C
  2. Completed Affidavit of Student Admission Form- Residing apart from Parent (one per student-needs notarization) 2-17-2018 Form E
  3. Power of Attorney (Must be signed by SISD Parent & Resident-needs notarization for both) 2017-2018 Form G
  4. Copy of enrolling parent/guardian's driver's license

 

 

From Sunnyvale ISD Resident:

  1. Completed Affidavit of Student Admission  Form -Residing apart from Parent (one per child-needs notarization) 2017-2018 Form E
  2. Copy of most recent property tax statement and a copy of current water, gas or electric bill
  3. Copy of Sunnyvale ISD resident driver's license

 

 

For student not living in Sunnyvale ISD Boundaries but Eligible to Attend Sunnyvale ISD under the TEA Grandparent After-School Care Clause (Board Policy FD LOCAL & LEGAL)

You must return the requested documents below to Sunnyvale ISD Student Services Department, 417 E. Tripp Road, Sunnyvale, TX 75182, before being able to enroll your child at the campus:

 

From Parent:

  1. Completed Grand Parent Affidavit (2017-2018) Pages 1 & 2 (one per child) Needs Notarization. Form F
  2. Copy of enrolling parent/guardian's driver's license

 

 From Grandparent:

  1. Copy of most recent property tax statement and a copy of current water, gas or electric bill in grandparent’s name
  2. Completed Grandparent Supplemental Form F (2017-2018) One per child, needs notarization
  3. Copy of grandparent driver’s license
